No. Jubilant provides mobile outpatient rehabilitation. Home health is a different model that may include nursing, aides, and other qualifying services in addition to therapy.
Referral pathways can involve the resident’s physician or other qualified provider, a community care team, the resident, or the family, depending on clinical and payer requirements. Protected health information should use Jubilant’s clinical referral channel, not the public inquiry form.
