Homebound required?

No. This is outpatient therapy delivered at home.

Confirmation

We confirm receipt the same business day.

• The patient doesn't qualify for home health, or home health has ended, but still needs therapy.
• Getting to a clinic is unrealistic because of transportation, fatigue, cognition or caregiver schedules.
• The patient needs more than one discipline, such as PT for walking and speech for swallowing.
• The patient lives in assisted or independent living and benefits from therapy in their own setting.

1. Receipt confirmed the same business day.
2. Fit check. We confirm location, payer and clinical fit, and call you if anything is missing.
3. Patient contacted to schedule.
4. Evaluation within 48 hours of referral.
5. Update to the referring provider with the evaluation, then progress notes every 10th visit or 30 days (whichever comes first), and a discharge summary.

One fax covers PT, OT and speech, so you don't coordinate three providers.

We call you if something is missing instead of letting the referral stall.
