Therapy in the resident's apartment or your therapy space. Our clinicians come to your building on a schedule that fits your routines.
Three disciplines, one partner. Physical therapy for strength, balance and walking. Occupational therapy for dressing, bathing and daily routines. Speech therapy for swallowing, communication and memory strategies.
Fast starts. Evaluations within 48 hours of referral, so a resident is not waiting a week after a fall.
Your team stays informed. We share the evaluation, a progress note every 10th visit or 30 days (whichever comes first), and a discharge summary with the people you designate, within HIPAA rules.
Families get a clear point of contact. We handle their questions about therapy, so your staff doesn't have to.
What it costs your community
Nothing. There is no fee to partner with Jubilant. We bill the resident's insurance directly: Medicare, Aetna, Blue Cross Blue Shield, Cigna or Curative. Residents can also pay privately.
How a partnership starts
- Fit call (15 minutes). We confirm your location, resident mix and what you want to improve.
- Walk-through. We visit, meet your nursing or resident-care lead, and agree on how referrals reach us.
- Balance-screening day (optional). A free group screening that helps identify residents who may benefit from therapy.
- First referrals. Residents are evaluated within 48 hours of referral.
- Ongoing updates. For every resident you refer, your team receives the evaluation, progress notes and a discharge summary.
